There are no guarantees on how long your child's physical will take. It usually runs very quickly and smoothly. Remember, this is a FREE gift to save you time, money, and gas. Patience and gratitude is appreciated.
Read Thoroughly:
Your child will get a Sports Physical packet from CCHS main office, Coach Harnly, or print them off from links below.
Parents must complete, sign, date your portion of the forms.
Your child will return the completed forms to Coach Harnly NO LATER THAN MAY 6 for pre screening with the Nurse. No exceptions.
